Retinopathy of prematurity (ROP) is an eye disorder caused by abnormal blood vessel growth in the light sensitive part of the eyes (retina) of premature infants. ROP generally affects infants born before week 31 of pregnancy and weighing 2.75 pounds (about 1,250 grams) or less at birth.

How long does ROP last?
If your child has mild retinopathy of prematurity (Stage 1 or 2), the abnormal retinal blood vessels usually heal on their own sometime in the first four months of life. But if the ROP worsens, he may need treatment.
Can babies with ROP see?
About 14,000 to 16,000 babies born in the United States each year have ROP. Most babies with ROP have a mild case and don't need treatment. But babies with severe ROP can have vision problems or blindness. About 400 to 600 babies each year become legally blind from ROP.
What is ROP test?
This test is done inside the neonatal unit or hospital, during a visit by the retina specialist or paediatric ophthalmologist. The pupils of the eye are dilated using drops and the retina is examined with an indirect ophthalmoscope for signs of ROP.

Only some babies with birth weights less than 1250 grams develop ROP and in most of these babies the ROP is mild and goes away without treatment. Only a very few babies develop severe ROP and treatment is successful in most cases.

Not all babies who are premature develop ROP. There are approximately 3.9 million infants born in the U.S. each year; of those, about 28,000 weigh 2¾ pounds or less. Stage 1 is the mildest form of ROP. Babies at this stage or stage 2 often don't need any treatment and will have normal vision. Babies with stage 3 have more blood vessels that are abnormal. These may be large or twisted, which means the retina could start to come loose.

The results of this meta-analysis suggest that vitamin A supplementation and early, aggressive PN may reduce any stage ROP in preterm infants. Risk of severe ROP or ROP requiring surgery may be reduced with breast milk feedings, vitamin E supplementation, and inositol therapy.
When is ROP screening done?
Initial screening should be performed at 31 weeks' postmenstrual age in infants with gestational ages of 26 6/7 weeks or less at birth, and at four weeks' chronological age in infants with gestational ages of 27 weeks or more at birth by an ophthalmologist skilled in the detection of ROP.

The golden standard of treatment in ROP is the laser photocoagulation of the non-vascularized retina. The most vulnerable period when ROP is at high risk of rapid progression is comprised between 34 and 35 weeks postconceptional age (PCA) and 36–37 weeks PCA.
How is ROP diagnosed?
The only way to determine if babies have ROP is to examine the inside of their eyes for abnormalities in the retina. Ophthalmologists trained in the diagnosis and treatment of ROP will examine your baby's eyes. During this exam, your baby's pupils will be dilated with eye drops so the retina can be studied.
